Summary:
Serve the community and Epworth by treating health needs for the surrounding community and centers under Epworth’s Institute for Child and Family Well Being, through direct care and preventative education. Abides by all Medicaid, Epworth, and SC DSS policy and procedures.

Primary Performance Objectives:

  • Ensures the flow and appropriateness of the day to day health care of independent living residents, foster children, as well as members of the community to include preventive health and wellness, health education, disease prevention and treatment in collaboration with the consulting psychiatrist, physician, dentist, school personnel, state and local health care agencies and ECH staff
  • Ensures the training and monitoring of staff performance in medication administration, universal precautions, First Aid/CPR/AED, and standing orders
  • Maintains records, charts, Community Health Center processes, and physicals in accordance with relevant state and local regulations, and provides client care that reflects the mission, vision, and values of Epworth and the essential principles of the C.A.R.E. practice model
  • Preform initial vitals and assessments on all patients for the Community Health Center as well as under the Institute.

Job Task Summary:

  • Performs medical procedures and manages medication administration to patients, residents and Foster Care children as ordered by the attending physician(s) within the scope of generally accepted nursing practices
  • Functions professionally as a member of the Epworth team, and is available for input as needed concerning client health
  • Maintains privacy and confidentiality of all patient and staff health information
  • May provide health related information on the prevention of communicable diseases; coordinate health programs with outside organizations and agencies (including ECH); provide general health related information and instruction to individuals, families, and community groups
  • Assess and evaluate all patient’s response to treatment and medication
  • Advise patients on available and appropriate medical and social services
  • Participates in the Epworth PQI process as assigned
  • Completes admission physical on new Independent Living residents within 48 hours of admission, and within 72 hours for new Foster Children. Completes annual physical for all community patients as scheduled
  • Maintains up to date medical records on each patient seen and treated.
  • Ensures copies of medical documents are sent to the correct file
  • Works with Epworth staff to ensure patients are actively enrolled in Medicaid
  • Serves as a resource to staff for health and safety related issues to include medication administration and potential side effects and interactions, first aid and psychosocial issues
  • Collaborates with the agency’s dietitian to ensure dietary needs are met
  • Ensures reporting and timely and accurate billing to applicable agencies to procure maximum reimbursement for billable health related services when appropriate
  • Responsible for ensuring that the Trenholm Campus buildings and centers are adequately equipped with health and safety supplies and equipment in a cost effective manner
  • Attends and participates in individual and group supervision meetings, calendar meetings, and other meetings as assigned.

Supervisory Responsibilities:

  • Direct supervisor to the Nursing Assistant (if applicable)
  • Collaborates with Epworth Program administration to provide a system for after hours on call support and consultation for medical concerns

Qualifications:

  • Nurse Practitioner preferred, BSN required
  • Current South Carolina Board of Nursing licensure
  • Certified in basic life support
  • Must have up to date personal liability insurance
  • Certified by the American Red Cross to teach CPR and First Aid
  • Minimum of five years’ experience as a licensed nurse
  • Experience as a community based nurse preferred
  • Must be able to pass standard background checks, including: drug screen, employment physical, SLED, DSS Central Registry and Sex Offenders Registry
  • Has a belief system and engages in faith practices that are compatible with those of the United Methodist Church
  • Must be proficient with various software programs such as Microsoft Word, Excel, and electronic communication. Know or have the ability to learn any database system required
