The links below are to forms, instructions and documents needed for you to complete an application for Commissioning or Ordination. Begin by reading the memorandum & checklist.  Look through the requirements and mark all deadlines on your calendar. Make sure you choose the correct instructions for your process. Some forms have two completion options from which to choose.  You only need to complete a form using one of the available options.

All submission file names should follow the following format: candidates first initial and last name, hyphen, subcommittee Initials.  Examples:

Due by January 10th

• For AM-PD-PE Candidates
  • Medical Report (F103)
  • Action Report: will be submitted on your behalf by your dCOM.
  • Official Transcript (sent to Clergy Services even if you are still in school)
• For PD-PE Candidates
  • Two Seminary Recommendations (Provisional Elder & Deacons Candidates Only)
    • Form for 1st Seminary Recommendation
    • Form for 2nd Seminary Recommendation
• For AM Candidates Only
  • Psychological Testing Results (results must be in our office by this date) (Associate Members Only)
  • SPPRC Recommendation Form (submitted to Clergy Services by your SPPRC)

Notes:

• A Final Seminary/COS Transcript showing graduation with M.Div. (confirm if in current permanent file) is required to be in your file before Annual Conference begins.
• NTERVIEWS WITH FULL BOARD FEBRUARY 53-4, 2025 in Myrtle Beach. The specific location will be sent to you at a later date.
• Letter to candidates from BOM Registrar setting date and time of interview approximately 3-4 weeks prior to meeting.
• FINAL APPROVAL AT ANNUAL CONFERENCE JJune 8-11, 2025 at The Greenville Convention Center, Greenville, SC: Must be approved by 3⁄4 majority vote of Clergy Session; Reception and Ordination Service at Annual Conference
