The links below are to forms, instructions and documents needed for you to complete an application for Commissioning or Ordination. Begin by reading the memorandum & checklist. Look through the requirements and mark all deadlines on your calendar. Make sure you choose the correct instructions for your process. Some forms have two completion options from which to choose. You only need to complete a form using one of the available options.

- FINAL APPROVAL AT ANNUAL CONFERENCE June 8-11, 2025 at The Greenville Convention Center, Greenville, SC: Must be approved by 3⁄4 majority vote of Clergy Session; Reception and Ordination Service at Annual Conference
